Position: HCC Coding Specialist

Work mode - Only Work from office
Roles & Responsibilities:
Review and analyze patient medical records to accurately assign HCC (Hierarchical Condition Category) codes
Ensure coded diagnoses are supported by proper clinical documentation in compliance with CMS and ICD-10-CM guidelines
Identify documentation gaps and collaborate with internal teams or providers for clarification
Maintain high accuracy, productivity, and quality standards
Follow client-specific coding policies and risk adjustment guidelines
Participate in internal quality audits and implement feedback
Stay updated on coding regulations, HCC model updates, and compliance standards
