ACEP Emergency Department Facility Coding – Level 2 (CPT 99282)

Overview

CPT 99282 represents an Emergency Department (ED) visit for a patient who requires an expanded, problem-focused history and examination, and straightforward medical decision-making. This level of care includes slightly more complex interventions than Level 1 and may involve basic diagnostic tests or simple treatments.

  • Type A: APC 613

  • Type B: APC 627

  • HCPCS: G0381

Possible Interventions

Could include interventions from previous levels, plus any of:

  • Tests by ED Staff (Urine dip, stool hemoccult, Accucheck or Dextrostix)

  • Visual Acuity (Snellen)

  • Obtain clean catch urine

  • Apply ace wrap or sling

  • Prep or assist with procedures such as: minor laceration repair, I&D of simple abscess, etc.

  • Discussion of Discharge Instructions (Simple)

Potential Symptoms/Examples Which Support the Interventions

  • Localized skin rash, lesion, sunburn

  • Minor viral infection

  • Eye discharge – painless

  • Ear pain

  • Urinary frequency without fever

  • Simple trauma (with no X-rays)

Key Notes for Coders

  • Level 2 visits typically involve one or two minor procedures or diagnostic tests.

  • Document both the interventions and patient symptoms to justify this level of service.

  • If the visit involves more complex testing or multiple reassessments, consider whether a higher-level code (e.g., 99283) is warranted.
